Breast shape changes over time. Skin loses elasticity and firmness through aging, pregnancy, breastfeeding, and weight change, and the breast gradually descends — a change called ptosis.
A breast lift, or mastopexy, raises, reshapes, and tightens the breast, removing excess skin and repositioning the nipple and areola to a natural height. Where the areola has stretched and enlarged, it can be reduced in the same operation to restore proportion.
One thing worth understanding from the outset: a lift repositions and firms what you have — it doesn’t add volume to the upper breast. If you want more fullness as well as lift, a lift can be combined with breast augmentation. If your breasts are larger than you’d like as well as sagging, a lift can be combined with breast reduction. Your surgeon will discuss which combination suits your goals.
Why Breasts Sag?
- Ageing: skin and supporting tissues gradually lose elasticity.
- Changes in skin and tissue quality over time.
- Weight gain: extra fat increases breast size and weight, pulling the breasts downward.
- Rapid or major weight loss: breast volume drops quickly, leaving excess skin.
- Pregnancy and breastfeeding: milk glands enlarge during pregnancy, then shrink after breastfeeding stops, leaving the breasts smaller and looser.
- Genetics: naturally large breasts tend to sag more easily.
- High-impact activity without enough support from a well-fitting sports bra.
Breast Lift After Pregnancy and Breastfeeding
- Wait at least 6 months after you stop breastfeeding, so milk production has fully stopped. Residual milk can leak into the surgical area and increase the risk of infection.
- The breast tissue should be fully dry, with no remaining milk.
- Ideally, plan a breast lift once you’ve finished having children. A future pregnancy can stretch the breasts again, and a lift may affect your ability to breastfeed. See the FAQ below.
- A breast lift is often part of a Mommy Makeover.
Degrees of Sagging and the Matching Technique
| Degree | What it looks like | Usual technique |
|---|---|---|
| Normal | Nipple sits in the center of the breast and points forward | No lift needed |
| Mild | Breast drops slightly and the nipple points a little downward | O-scar, if a lift is wanted |
| Moderate | Breast clearly droops and the nipple points downward | Keyhole scar, or an O-scar combined with an implant |
| Severe | Breast hangs low, sometimes almost to the navel, with the nipple pointing down | Inverted T-scar |
Incision Options
The incision pattern depends on breast size and shape, the size and position of the areola, the degree of sagging and skin elasticity.
1. Periareolar Lift (O-Scar)
The surgeon marks the ideal areola size, removes a ring of skin around it, tightens the skin and tissue, and closes the incision around the edge of the areola.
- Advantage: minimal visible scarring, as the scar sits around the areola only
- Limitation: suitable only for mild to moderate sagging
2. Vertical Lift (Keyhole Scar)
An incision around the areola continues vertically down to the breast crease. Excess skin and tissue are removed along this line to lift and reshape the breast.
- Advantage: restores a firm, well-shaped breast
- Limitations: suitable for moderate sagging; the vertical scar is more visible than an O-scar
3. Anchor Lift (Inverted T-Scar)
An incision around the areola, a vertical line down to the crease, and a curved incision along the breast crease form an inverted T. This allows a large amount of skin and tissue to be removed, so it is used for severe sagging or when the breast also needs to be reduced.
- Advantage: suitable for breasts of any size and degree of sagging
- Limitation: the most visible scar of the three, although the horizontal part sits in the breast crease
Who Is a Good Candidate?
You will be the right candidate if you have
- Feeling your breasts sag or lose shape and volume
- A flatter breast, elongated shape, or pendulous
- The nipples fall below the breast crease
- The nipples and areolar point downward
- Stretched skin and enlarged areolar
- One breast is lower than the other.
- Physically healthy and stable weight.
Preparation
- Consult your surgeon about your goals. If you’re combining the lift with augmentation, you’ll also choose a suitable implant size and shape.
- Have a physical examination, blood test and chest X-ray. If you’re over 40, an EKG and cardiac stress test are required before general anesthesia.
- If you take hormones, your doctor may ask you to stop them 1–2 weeks before surgery.
- Stop aspirin, anti-inflammatory drugs and herbal supplements that may increase bleeding at least 2 weeks before surgery.
- Stop smoking at least 2 weeks before and after surgery, to protect blood supply to the healing skin.
The Procedure
- Marking: the incision pattern is chosen according to the degree of sagging and the nipple position, and the surgeon marks it on the skin.
- Anesthesia: the operation is performed under general anesthesia.
- Lifting and reshaping: the breast tissue is lifted and reshaped, excess skin and tissue are removed, and the nipple and areola are moved to a natural, higher position. Enlarged areolas are reduced.
- Closure: the inner breast tissue is shaped with absorbable sutures, and the skin is closed with sutures or medical tape.
Post-operative Care
The post-operative care for a breast lift is similar to breast augmentation to minimize any post-operative discomfort after surgery can be done as follows;
- You should wear a compression bra for extra support and positioning to prevent fluid accumulation and control swelling.
- Sleeping with torso slightly elevated to control bruising and swelling
- The dressing will be applied to the incisions until stitches are off seven days after surgery.
- Keep clean and dry the incision every day.
- Take medicine for pain relief.
- Take stitches off seven days after surgery.
- You will need a follow-up for every appointment
- Avoid strenuous activities for at least two weeks
Recovery
- O-scar lift: mild discomfort for a few days. Most patients return to work after 2–3 days.
- Keyhole or inverted T-scar lift: most patients return to work after 4–5 days. Pain, tightness, swelling and bruising are normal, and numbness around the incisions may take several months to resolve.
- Lift combined with augmentation or reduction: recovery takes longer, usually about a week before returning to work.
Avoid strenuous exercise, arm-intensive workouts and heavy lifting for at least 6 weeks. Wearing a sports bra helps you heal and keeps the shape in place.
Risks and Complications
- Scarring, including raised or keloid scars
- Bleeding, bruising and swelling
- Pain or itching around the incisions
- Delayed wound healing
- Asymmetry between the breasts
- Infection: warmth, redness and swelling of the breast with a fever. Contact your surgeon promptly if you notice these.
- Reduced or lost sensation in the breast or nipple
- Capsular contracture (hardening around the implant), only if an implant is placed at the same time
- Risks from anesthesia

Choosing the Right Surgeon for a Breast Lift
A breast lift needs more experience than a standard breast augmentation, because the surgeon must reshape the breast tissue and position the nipple accurately, not just add volume. When choosing a surgeon:
- Consult a board-certified plastic surgeon
- Check the surgeon’s credentials and qualifications
- Review before-and-after photos of the surgeon’s previous patients
- Ask for recommendations from friends, family or your doctor

What Questions Do Patients Commonly Ask?
What is a breast lift ?
A breast lift, also known as mastopexy, is a surgical procedure that has been designed to raise and reshape breasts that have begun to sag.
It’s a popular option for women who feel their breasts have lost their youthful firmness and perkiness due to factors like aging, pregnancy, breastfeeding, weight fluctuations, and indeed gravity.
By removing excess skin and tightening the surrounding tissue, a mastopexy can restore a more youthful and uplifted breast profile and boost confidence.
Will a breast lift make my breasts bigger?
No. A breast lift raises and reshapes the breasts but doesn’t add volume, and it may not fill the upper part of the breast. If you’d like more fullness, your surgeon can combine the lift with breast augmentation using implants. If your breasts are too large, a lift is usually combined with a breast reduction.
Can I breastfeed after a breast lift?
Many women can, because the nipple usually stays attached to the underlying breast tissue during a lift. However, some milk ducts may be affected, so breastfeeding can’t be guaranteed. If you plan to have more children, it’s often best to wait, as pregnancy can also stretch the breasts again.
How long will the results last?
The lift itself is long-lasting, but your breasts will continue to age naturally. Significant weight changes, pregnancy and gravity can all cause some sagging to return over time. Keeping your weight stable and wearing a supportive bra help maintain the result.
What is the difference between a breast lift and breast augmentation ?
While both procedures can enhance the appearance of your breasts, they address different concerns:
- Breast Lift: Focuses on lifting and reshaping sagging breasts.
- Breast Augmentation: Focuses on increasing breast size and fullness.
Depending on your goals and desires, you may benefit from one or both of these procedures.
Will the scars be noticeable?
Every breast lift leaves scars, but they’re placed around the areola, down the lower breast and in the breast crease, so they’re usually hidden by a bra or swimwear. Scars are pink at first and gradually fade over 12–18 months. If you’re prone to raised scars, tell your surgeon, as early scar treatment can help. Thick scars can be treated with keloid removal.
Can a lift and implants be done in the same operation?
Often, yes, and many patients prefer to recover once. In some cases of severe sagging, your surgeon may recommend doing the lift first and placing implants in a second stage, for a more predictable and safer result. If you already have implants, a lift can be combined with breast implant revision, as in Lisa’s case above.
Why do patients travel to Bangkok, Thailand for breast lift surgery?
Bangkok has become a well-established destination for breast surgery, with surgeons who perform all three lift techniques — periareolar, keyhole, and inverted T — routinely rather than defaulting to one approach.
At Kamol Hospital in Bangkok, each case is planned around your anatomy and skin quality, which matters for a procedure where incision choice is largely a matter of surgical judgment.
How long should I stay in Bangkok after a breast lift?
Plan to stay in Bangkok for about 7–10 days, until your stitches are removed on day 7 and your surgeon has checked your healing. If your lift is combined with augmentation, reduction or implant revision, allow a little longer. Thailand is one of Asia’s leading destinations for breast surgery, and Kamol Trip can arrange accommodation close to the hospital.
