Prepare a Medical History Report
Your medical history report is a record of previous treatments from physicians and hospitals. Reports may vary in details and length, depending on the extent of your medical history.
A psychological evaluation is also required in order to determine if there are any mental health concerns that could influence an individual’s state of mind and to assess a person’s readiness to undergo the physical and emotional trauma during the transitional process. A letter from psychiatrist must be presented after the evaluation.
1. Prepare a Medical History Report
Your medical history report is a record of previous treatments from physicians and hospitals. Reports may vary in details and length, depending upon the extent of your medical history. But all reports generally contain common details and information, such as physical examination as well as focused pre-anesthesia physical examination that includes an assessment of the airway, lungs, and heart with documentation of vital signs. Other medical information such as vision, hearing, and nervous system responses may also be required. Unexpected abnormal findings on the physical examination should be investigated before your elective surgery.
General Tests
- Complete Blood Count (CBC)
- Electrolytes
- Anti-HIV
- Hepatitis screening
- Chest X Ray
A blood test will be performed to check your overall health, and you will likely have an electrocardiogram – an EKG, where electrodes placed on your chest – to measure your heart function. If there are any concerns about your lungs, you may be required to have a chest X-ray.

3. Visa

Travelers from the United States and Canada may enter Thailand exempt from any visa requirement for a period of 30 days under the conditions as follow.
